An excimer laser is a type of laser that has been used in ophthalmology and dermatology for many years. It works by removing tissue with the use of oxygen, nitrogen, or carbon gases as an excitation medium. These are also known as gas lasers and belong to a general group called argon-ion lasers. The way this process occurs is through photodissociation which breaks down molecular bonds between atoms so they can be removed from living cells without harming them.

 

Femtosecond Ophthalmic Laser:

Femtosecond Ophthalmic Laser is a new laser technology that was developed to provide benefits of the Excimer and Intense Pulsed Light (IPL) technologies but without some of their drawbacks. Femtosecond lasers are used for many ophthalmological procedures like cataract surgery, retinal disease treatment, corneal transplantation, or diagnosis.

The femtosecond laser uses light pulses with a duration in the range from 0.001–0.002 seconds which allows less tissue damage than traditional lasers because they avoid thermal injury by heating up cells slower than other laser types.

Refractive Surgery:

Excimer lasers can be used in the following ways-

  • to correct myopia (nearsightedness) and hypermetropia (farsightedness).

  • in treating corneal irregularities such as keratoconus, pellucid marginal degeneration, or post-refractive keratectomy scars.

 

Cataract Surgery:

Femtosecond ophthalmic laser is a newer technology that enables surgeons to perform cataract surgery without using an incision on the eye’s exterior, which could lead to infections. It has been found that this technology offers improved safety with less disruption during surgery and it results in less post-operative pain than conventional techniques.

 

Capsulotomy:

A capsulotomy is a surgical procedure to open the capsule of an eye, which involves removing pieces of it using a laser. The process may also be known as capsular stripping or anterior capsular cataract surgery (ACCS). This technique is used in many intraocular lenses implantation procedures and can help with the placement inside the eye without damaging delicate tissues.

Excimer lasers are often implemented for this purpose because they have high-energy photons that can remove superficial layers from hard materials due to their short wavelength. Femtosecond ophthalmic lasers are not typically used during Capsulotomy because they operate at lower power levels than excimer lasers.

 

Trabeculoplasty:

Trabeculoplasty is a surgical procedure used to treat glaucoma by decreasing the pressure inside the eye. It may involve cutting and removing part of semi-transparent tissue in order to decrease fluid production or reduce abnormal meshwork on the surface, often both.

Trabeculectomy can also be done through an incision made underneath one eyelid (subconjunctival). The excimer laser can be utilized as a treatment for trabeculoplasty where it has been shown that its use reduces intraocular pressure after surgery indicating that there are fewer drainage channels blocked preventing fluids from escaping which therefore decreases pressure on the retina cells resulting in lowering retinal edema postoperatively.

 

Diagnostics:

Excimer and femtosecond lasers are the most commonly used in ophthalmic diagnostics. They are also widely used to test for corneal disorders, such as keratoconus or cataract-related changes, which cause defects on the surface of the cornea.
