ECMO refers to Extracorporeal Membrane Oxygenation, and it is a treatment for severe respiratory failure. A machine that provides ECMO removes the oxygen-less blood from an individual's body and pumps it through a membrane giving them oxygen. Until now, this has been done only in very few hospitals worldwide because of its complexity--that’s changing with the introduction of more efficient devices.

An ECMO device is a life-supporting machine that acts as an artificial lung and heart. Theoretically, it could be used to treat any condition in which the organs are failing, such as multiple organ failure or severe sepsis. In practice, however, ECMO has not been shown to help people with irreversible brain injuries or who have suffered strokes.

On the basis of type, ECMO devices are segmented into VV ECMO, VA ECMO, and AV ECMO.

VV ECMO

VV ECMO refers to venovenous extracorporeal membrane oxygenation. It is a type of Extracorporeal Membrane Oxygenation (ECMO), which is an extracorporeal circulation system that uses artificial means to pump blood from the venous system, through the pulmonary bypass circuit and then back into the arterial system. A VV device pumps blood from the body into a membrane outside of it to increase oxygenation in areas that would otherwise not receive enough air.

VA ECMO

A vacuum-assisted ECMO device is a type of extracorporeal membrane oxygenation that uses an artificial pump to create and maintain the blood flow necessary for survival. It provides cardiopulmonary support by withdrawing venous blood, removing carbon dioxide from it, and returning oxygenated blood back into the body via two cannulas inserted in large veins.

A VA ECMO device works when the patient's oxygen-depleted blood is pumped from a vein near the heart to an artificial lung, called a membrane oxygenator, where it picks up the fresh air and returns to circulate through the body. When used as cardiopulmonary support during surgery in place of life support machines such as ventilators, this type of system is known as extracorporeal circulation (ECC). The person who needs help with breathing can wear only one mask connected to both types of apparatus. A VA device sends only venous return (deoxygenated) back through an external membrane by way of gravity or vacuum pressure for re-oxygenation purposes; this is usually used on patients with pulmonary hypertension who are too weak to breathe effectively on their own.

VA ECMO devices are capable of being mounted on rolling platforms or ruggedized for transport by military personnel. It allows medical professionals easier mobility and access than simply carrying around another machine that weighs more. VA ECMO devices are the most logical choice for military personnel and civilians due to their portability, durability, and low cost. Virtually all VA ECMOs have at least two types: single-lung systems (SL).

AV ECMO

An AV ECMO is a device that provides respiratory support to patients with severe lung failure. This type of ECMO system can be used as an alternative treatment for neonatal infants, children, and adults who are unable to breathe on their own because of acute respiratory distress syndrome (ARDS). An AV system does both simultaneously: It removes deoxygenated blood from the body and returns it with extra oxygen after passing through a filter inside a membrane outside of the patient's body--this is often called "extracorporeal circulation.

Hospitals

An ECMO device can provide respiratory support to patients with severe lung failure. It works by taking blood from a patient and pumping it through an artificial heart or mechanical pump outside of the body, which removes deoxygenated blood and returns it with oxygen after passing through a filter inside a membrane called "extracorporeal circulation." This type of ECMO system is often used as an alternative treatment for neonatal infants, children, and adults who are unable to breathe on their own because of acute respiratory distress syndrome (ARDS).

Diagnostic Centers

ECMO devices are often utilized in diagnostic centers for longer procedures. VV ECMO devices can be attached to a patient and then the physician, with assistance from an anesthesiologist, can use it during surgery without having to stop operating or using instruments on another part of the body.

Clinics

ECMO devices are used in clinics to provide respiratory and circulatory support for patients with cardiac or pulmonary complications. ECMO is a life-saving procedure if performed early enough. It offers an alternative option when other treatments have failed, such as mechanical ventilation or surgical intervention. The device is used for the treatment of respiratory failure, shock, and cardiac arrest. In addition, it also can be applied to patients with acute myocardial infarction or septic shock among others.

Ambulatory Surgical Centers

The application of ECMO devices in ambulatory surgical centers is growing. The demand for these types of facilities has grown due to the need for minimally invasive surgeries, which have been less risky and more cost-effective than traditional surgery methods. Patients who undergo this type of treatment are at a lower risk of infection because there’s no skin incision made by instruments like a scalpel or laser knife.
